Download the dlgdiag_5_21.zip.
Extract the dlgdiag5.exe file and copy to a DOS bootable diskette.
Boot from this diskette. Make sure that your boot order in your Bios is set to boot from the floppy drive first.
At DOS prompt type dlgdiag5.exe to run the program.
